= All you need to know if your child is on target

 United Way wants every child less than five years old, who lives in the Greater Chattanooga Metropolitan area, to be screened with an Ages and Stages Questionnaire at least once a year.

Call United Way at 423-752-0302 to get your free Ages and Stages Questionnaire for every child in your home who is less than five years old.  Fill it out and send it back in the enclosed self-addressed, stamped envelope, and we'll send you a free family pass to the Creative Discovery Museum for every completed ASQ you return. You can also e-mail us at iicintern@uwchatt.org with your name, mailing address and names and ages of your children who qualify for the ASQ.

Why do we want you to do this? Because an Ages and Stages Questionnaire is a fast, easy and free way for parents to monitor their child's development. It lets them know their child is developmentally on target.

That's important because education experts know that if a child has a developmental delay or a learning problem, it is more likely to be corrected if caught early and treated promptly - before becoming something much bigger that can’t be easily remedied.

Children who begin Kindergarten ready for school are more likely to be successful in school and life, and stopping problems before they start is more efficient and successful than letting things get out of hand. 

The great thing about this screening is that parents can do it in the comfort of their own home at their convenience.
